The founders, Dr. Adeebur Rahman MD, Dr. Rajesh Iyengar MD, and Dr. Asad Zaman MD, each having over 15 years of experience in Internal Medicine with a focus on geriatrics and post-acute care, identified a concerning gap in care for their patients as their medical conditions changed.
To avoid fragmented care for their patients, the founders established a specialty care group which includes outpatient clinics, hospital affiliations, post-acute rehabilitation, and house-call services to promote the continuity in their patients care and ultimately enhance patient outcomes.
